🏥 How to Obtain Your Medical Certificate

TTC requires a Medical Certificate of Physical Fitness from a Registered Medical Practitioner (MBBS or above). Here is the recommended process:

Visit a Government Hospital / PHC
This is fastest and lowest-cost. Bokaro General Hospital (BGH) is ideal for applicants in the region. Government MBBS doctors can issue fitness certificates for educational institutions.

Print the Medical Form below and carry it to the doctor
Use the print button below. The doctor fills, signs, dates, and stamps the printed form.

Request a General Physical Fitness Examination
Covering: ENT, Eyes, Cardiovascular System, Respiratory System, Abdominal Examination, and Central Nervous System.

Disclose all medical history honestly
Including: Jaundice, Tuberculosis, Epilepsy, Rheumatic/Heart conditions, Respiratory problems, Congenital disorders, or any other illness.

Ensure the certificate has doctor's signature, stamp, registration number, date, and address
A certificate without the MCI/State registration number is not valid. Keep a photocopy for yourself.

Include the original signed certificate in your physical application envelope
Include it with your full application packet sent to TTC.
